Anyone have any idea why a 5 speed turbo box would be noisey in 4th an 5th gears only?
Apparently there is no noise in 1st, 2nd or 3rd.
The owner also says that it is sometimes difficult to select 1st gear.

My guess is that he has a clutch/flywheel problem rather than a gearbox problem.

He did his own conversion, and I haven't personally seen the vehicle since this was done, so can't give a better description.

Bruce I had a Nissan 4WD with similar problems, it turns out the box has a 5th gear which has a so-called idler gear, this idler gear has weak bearings and this causes the rumble. As the 5th and idler are a matched set you have to replace both gears, bit
